Abstract

Background

We sought to evaluate the use of oral administration of sildenafil citrate (Viagra) by men aged 18 to 65. Patients were randomly assigned to receive either sildenafil citrate (n = 60), placebo or placebo-containing doses for 4 weeks. The primary outcome was the change in systolic blood pressure (SBP) from baseline to endpoint in patients receiving sildenafil citrate. The secondary outcomes were change in systolic blood pressure (SBP) from baseline to endpoint, and change in HDL cholesterol, total cholesterol, triglycerides, total-fat, free-fat free-lipid and HDL cholesterol levels from baseline to endpoint. The primary analysis included 24,723 patients (ages 18 to 65) with ED at baseline and 24,723 patients (ages 18 to 65) with ED at endpoint. All patients were evaluated by investigator assessment at baseline and endpoint, and data were analyzed by univariate and multivariate analyses. In addition, the secondary analysis was conducted by using secondary outcome measures for the patients treated with sildenafil citrate. There was a significant difference in the change from baseline to endpoint in the sildenafil citrate group (3.12 mmHg vs. 1.84 mmHg, P = 0.001) and the placebo group (3.12 mmHg vs. 1.84 mmHg, P = 0.001). There was a significant difference in the change from baseline to endpoint in the sildenafil citrate group (2.20 mmHg vs. 2.06 mmHg, P = 0.002) and the placebo group (2.20 mmHg vs. 2.06 mmHg, P = 0.002). These findings indicate that sildenafil citrate was more effective than placebo for the treatment of erectile dysfunction in men. However, the effect of sildenafil citrate was not significant different from placebo (3.12 mmHg vs. 2.06 mmHg, P = 0.19).

IntroductionSildenafil citrate is an oral phosphodiesterase-5 (PDE-5) inhibitor which is indicated for the treatment of erectile dysfunction (ED). The mechanism of sildenafil citrate has been studied extensively, with the most well-documented being the reduction of nitric oxide (NO) in the corpus cavernosum, which is responsible for penile erection. Sildenafil citrate is the generic name for a class of PDE-5 inhibitors, which are used in the treatment of ED. In the literature, there are no data regarding the use of sildenafil citrate in the treatment of erectile dysfunction, as no significant differences in the safety of sildenafil citrate were found between patients receiving placebo and those receiving sildenafil citrate.

In the past, there have been reports of several cases of men using sildenafil citrate for the treatment of erectile dysfunction. The reported adverse reactions included headaches, flushing, upset stomach, nasal congestion, dizziness, vertigo, and urinary retention. There have been several case reports of men experiencing adverse events such as priapism, erectile dysfunction, and an erection lasting longer than four hours. Sildenafil citrate was the first of the PDE-5 inhibitors to be approved for the treatment of erectile dysfunction in the United States, and was used worldwide since its approval. Although sildenafil citrate was approved in the United States in 1999, its use was restricted due to high costs. The U. S. Food and Drug Administration has approved sildenafil citrate for the treatment of ED in 2004, and its use has been suspended since that time. In addition, sildenafil citrate has not been approved by the European Medicines Agency for use in the treatment of ED in the U. The FDA has approved sildenafil citrate as the first-line treatment for erectile dysfunction in men with ED. There have been reports of a significant increase in the number of men who develop erectile dysfunction in the past decade, with a median age of 67 years, and there has been a marked increase in the prevalence of erectile dysfunction in the United States from 2001 to 2009. Several reports indicate that there is a significant risk of death associated with ED and the risk of serious adverse events from the use of sildenafil citrate, including priapism, and that these events should be minimized. There is no specific treatment for ED and, therefore, there is no single treatment available for the treatment of ED.
