Background

Sildenafil (Viagra®) is an active ingredient that is widely used to treat erectile dysfunction (ED) and pulmonary arterial hypertension (PAH). The median L/R shunt was 0.5 ± 0.7 mm in the group 1 and 3.0 ± 1.6 mm in the group 2, which were statistically significant (P<0.05). The patients in the two groups had significantly higher mean L/R shunt (SD = 3.5) and mean L/R systolic blood pressure (SD = 1.5) than the patients in the other two groups (= 0.05). No adverse events were observed in the patients.

Conclusion

In the present study, Viagra® was associated with endothelial NO (NO) in the pulmonary circulation of patients with PAH. Moreover, the increase of NO levels in the pulmonary circulation of patients with PAH was significantly higher than that of the patients in the other two groups (= 0.005). Therefore, Viagra® should be prescribed in patients with PAH and with ED if the patient is already at a low risk for cardiovascular events.
