The current study was a retrospective study of the electronic medical records of patients admitted to the University Hospital of the First Affiliated Hospital, Sichuan, from June 2010 to June 2017. The study included data on all patients with PAH who had undergone a radical surgical procedure within the previous 7 years. The patients were divided into two groups, namely group 1 (n = 21) and group 2 (n = 25).
All patients were recruited from patients admitted to the Hospital of Medicine, Sichuan State University (China), from the Department of Cardiology, Sichuan Medical University, China, between June 2010 and June 2017. The patients who received Viagra® and received no treatment for ED were excluded. The patients with pulmonary hypertension were included in the study if they were aged <50 years old, and those with a left-to-right shunt (L/R shunt) were excluded. Patients with a previous history of cardiovascular events, history of cardiovascular disease, and previous history of coronary artery disease were excluded. In addition, patients with a previous history of stroke and heart attack were excluded. The patients with PAH who had undergone a radical surgical procedure within the previous 7 years and were excluded were also excluded. A total of 653 patients were enrolled in this study.
The patients who were admitted to the hospital with a history of PAH and had undergone a radical surgical procedure in the previous 7 years were divided into two groups: Group 1 (n = 21) and Group 2 (n = 25). The patients were excluded after the patients were excluded for any reasons. The patients were enrolled in the study in a manner approved by the Ethics Committee of the First Affiliated Hospital of Sichuan State University (China) and the Ethics Committee of the Second Affiliated Hospital of Sichuan State University, China (No. 2017-079-001).
The mean duration of follow-up was 8.4 ± 4.7 months, with a median of 7.0 months. The data were obtained from the patients who underwent the surgery, and the results were analyzed by using the Chi-square test. The statistical analyses were performed using the SPSS Statistics software version 20.0 (SPSS Inc., Chicago, IL, USA).
The mean age of the patients in the two groups was 44.2 ± 3.6 years (median = 45.8 years), and mean L/R shunt (SD = 0.7) was 31.3 ± 3.4 mm. The median L/R shunt was 0.5 ± 0.7 mm in the group 1 and 3.0 ± 1.6 mm in the group 2, which were statistically significant (P<0.05). The patients in the two groups had significantly higher mean L/R shunt (SD = 3.5) and mean L/R systolic blood pressure (SD = 1.5) than the patients in the other two groups (= 0.05). No adverse events were observed in the patients.
In the present study, Viagra® was associated with endothelial NO (NO) in the pulmonary circulation of patients with PAH. Moreover, the increase of NO levels in the pulmonary circulation of patients with PAH was significantly higher than that of the patients in the other two groups (= 0.005). Therefore, Viagra® should be prescribed in patients with PAH and with ED if the patient is already at a low risk for cardiovascular events.
